INSTRUCTIONS FOR THE DAY OF DENTAL SURGERY

1. Nothing to eat or drink by mouth as instructed by Dr. Sophia Tan, DDS.
2. Sign back portion of Informed Consent stating last time you or you child ate/drank.
3. Adult patients:  Medications normally taken on a daily basis should be taken on the day of surgery with a sip of clear liquid (not more than a teaspoonful) unless otherwise instructed by Dr. Sophia Tan, DDS.
