PRE-OPERATIVE INSTRUCTIONS
EATING OR DRINKING
FAILURE TO STRICTLY FOLLOW THESE INSTRUCTIONS COULD RESULT IN ASPIRATION AND MAY BE FATAL.
For your anesthesia, it is extremely important that patients have an empty stomach for their procedure. Please follow these instructions carefully.
Under the age of 5
- No solid foods for six (6) hours prior to appointment.
- Total of 4 oz. of water or apple juice ONLY may be taken up to four (4) hours prior to appointment time.
- Nothing may be taken by mouth for four (4) hours prior to appointment time.
CLOTHING
- Please wear comfortable clothing that includes a short sleeve shirt.
- Children should bring a light to medium blanket for the procedure and ride home.
- Please bring a change of clothing for children who do not wear a diaper or pull-up.
- Do not wear makeup, nail polish or false eyelashes.
- Please remove contact lenses prior to your appointment.
CHANGE IN HEALTH STATUS AND MEDICATIONS
- A change in health status, especially the development of a cold or fever, is extremely important. Please notify Dr. Sophia Tan, DDS immediately if there is any change in your child’s or your health status.
- Prescription medications should be taken as scheduled unless previously indicated by Dr. Sophia Tan, DDS and may be taken only with minimal sip of water.
DESIGNATED DRIVER
- A responsible adult must accompany the patient o the office and remain throughout the appointment.
- Do not plan on driving or making decisions for the next twenty-four (24) hours after the administration of anesthesia.
- Please arrange to have a responsible adult spend the rest of the day with you or your child.
- Please provide car seats for your children.

♥ POST-OPERATIVE INSTRUCTIONS
POST-OPERATIVE INSTRUCTIONS
ACTIVITY
- Observe patient for remainder of the day or until fully recovered. The effects of sedation or anesthesia will usually disappear within 24-36 hours.
- Limit patient to indoor, quiet activities until the effects of the anesthetic have subsided completely.
- Judgment may be impaired during this time, so do not allow your child to swim, bike ride, or play with other children. Place a soft blanket on the floor for your child to sleep and observe him/her closely.
DIET
- Today – give clear (“see-through”) liquids (e.g. apple juice, jello, clear soup, 7-up, Gatorade). Once clear liquids are tolerated, progress to a soft diet (e.g. applesauce, scrambled eggs, mashed potatoes, and soups) after 4-5 hours.
- Tomorrow – maintain soft diet then progress to regular diet as tolerated.
- NO dairy or citrus products for 24 hours after dental treatment.
- If teeth were extracted, DO NOT use a straw.
- If your child is not hungry, do not force him/her to eat, but encourage as much liquids as possible for the next 24 hours.
PAIN/FEVER MANAGEMENT
- When a patient has had a full mouth dental rehabilitation, 2 to 3 days of discomfort is to be expected. Muscle aches and a sore throat may occur, similar to the flu. Careful oral hygiene must be observed during the healing period.
- Acetaminophen (Tylenol) or Ibuprofen (Advil) are usually effective for pain management and should be taken at the first sign of pain according to dosage as recommended. DO NOT TAKE IF ALLERGIC TO THE ABOVE MEDICATIONS.
- It is common for children with a fever of up to 101 degrees Fahrenheit to develop or the first 12 hours. Tylenol elixir ever 3 to 4 hours as directed on box with plenty of liquids will tend to alleviate this condition, as well as any pain.
- If temperature exceeds 102 degrees Fahrenheit, please call Dr. Sophia Tan, DDS at 503-680-4406 immediately or bring your child to the nearest emergency room.

PATIENT INSTRUCTIONS PRIOR TO APPOINTMENT DAY
- Please complete, sign and return the following to Dr. Sophia Tan, DDS:
- Patient Health History
- Informed Consent
- Request for Medical consultation form – sign in box only please
- Financial Agreement
- HIPPA/Communication Form
- Please read and initial each section of the Informed Consent. If you have any questions, please note them and Dr. Sophia Tan, DDS will discuss them with you.
- The last section is to be filled out and signed the day of surgery.
- Please review the Pre/Post-Operative instructions.
- Your dental office will initially review the financial agreement with you, as well as Dr. Sophia Tan, DDS.
- Patient must be well 7-10 days prior to his/her appointment. No coughing, runny nose, fever, cold or flu like symptoms. Please notify Dr. Sophia Tan, DDS immediately at 503-680-4406 if the patient’s health status changes.
- It is advised that medications prescribed by the operating dentist for post-operative care are obtained from the pharmacy prior to the day of you appointment.
- There may be a request for a visit to your physician’s office for a pre-operative evaluation as well as blood work to be taken.
INSTRUCTIONS FOR THE DAY OF DENTAL SURGERY
- Nothing to eat or drink by mouth as instructed by Dr. Sophia Tan, DDS.
- Sign back portion of Informed Consent stating last time you or you child ate/drank.
- Adult patients: Medications normally taken on a daily basis should be taken on the day of surgery with a sip of clear liquid (not more than a teaspoonful) unless otherwise instructed by Dr. Sophia Tan, DDS.
