We do not bill your insurance directly and it is your responsibility to request reimbursement from your insurance carrier.  At this time Dr. Tan may only treat patients who are cash pay or have private insurance. She is not contracted with any state programs suchas Medicaid, OHP or Healthy Kids.
However, keep checking with us, we hope to be expanding our networks soon. From experience, most medical and dental insurances do not cover general anesthesia for dental procedures in office.  If you wish to bill your medical and/or dental insurance, forms will be provided for you with correct billing codes.  Insurance companies have contractual benefits with its subscribers and members only; therefore, we DO NOT guarantee what your insurance company will pay. We highly recommend contacting your insurance provider prior to your procedure for specific questions regarding your plan and coverage.

Dental restorations will be billed by your dental office according to their office policy and does not include Dr. Tan’s fees for general anesthesia.
